Dawn of Dreams. It had a faster pace than the previous games, which was one of the things I enjoyed the most about it.
Interesting, this was a topic of thought for me a few days ago. :P
Do I still play it? No. Do I still like it? Yes. The fourth installment would be a welcome sight indeed.
Shinobi (2002). I hit a wall once I reached a level in which the player had to essentially wall run and dash between enemies in mid-air thoughout the course of almost the entire stage. If levels of a similar nature existed beyond that point, I never got to see them - I left the game unfinished. :P
That, and Agile Warrior: F-111X, a PSone arcade flight sim. Consider yourself to be doing well, should you survive any of the opening missions long enough to hear the BGM track's second loop start.

The first game that comes to mind is V2000 (or Virus 2000), a game released on PC and PSX in 1998 in which the player uses a vehicle capable of transforming into a hovercraft or VTOL aircraft. After clearing up a bunch of oversized insects, spiders and plants, the player ends the stage by attacking and then decending into a purple dome-like structure referred to as a 'hive'.

Assuming it wasn't V2000, can you remember which platform(s) the game was released on, or which studio developed it?
Just Doom 3: BFG Edition and Under Defeat HD at the moment. Might pick up Killzone HD and Dishonored soon.
