A similar thing happened to me. I noticed that if you don't pick insane mode right after you unlock it, it disappears. To get it back load a previous game and then beat it ( i had a save from before the final boss) and the beat it, it will then be there. Good luck. :)

I'm one of the people that think this movie is gonna be full of fail because of its cast. I mean an almost all white cast for a movie like this? I dont mean to be a rascist but *sigh*
