So, you install the DLCs and when you reboot your PS3 you can't find them on Game data utility? That's kinda weird, maybe you should contact Sony. (Unless I didn't understand your problem?)
I forget who lived and who died, but I know it was about half and half. I was just glad Kasumi lived, as she is an awesome character. Out of curiosity, how do you get your entire team to live?

You have to buy all the ship upgrades and in the final missions you have to choose the correct people in the team assignments they give you. (team leaders, engineers etc.)
I had everyone survive in all my playthroughs. I considered going for a "nobody survives" playthrough, but I just couldn't do it. I kinda got attached to these guys lol. So, I just watched the bad ending on youtube.
The fact that you know the number of chapters isn't a spoiler. Unless I didn't understand your question. The game is amazing, imo anyway. Give it a chance.
First of all have you played the previous titles in the series? If not you might (well not might, you'll definitely) have problems understanding what's going on in the story. But other than that it doesn't matter that you're late. The game is amazing no matter what. It's the single player that makes MG what it is. Not online. Although there might still be people playing. I never really got into it.

Well I don't know if this can damage your PS3 since I always quit the game before turning off the console. But ejecting the game while it's running doesn't sound like the best choice to me.
Try holding down the button for a few seconds at startup until you hear the second beep. It just needs syncing. At least that's what had happened to me once.
