The Steam Icon File is actually a .VPK file, which is an all-inclusive installation file. Navigate to your C:\Program Files (x86)\Steam\steamapps\common\left 4 dead 2/left4dead2 folder and drop the VPK you downloaded into the addons folder. At this point, your custom campaign should be recognized automatically upon startup of L4D2 and if it is not listed in your Extras, that means the campaign was compiled incorrectly.
While I haven't played Borderlands, I am using Windows 7 Ultimate 64 Bit and the G92 8800GTS 512MB with 195.62 driver, and I haven't had any problems. It might just be you or I could just be lucky.
