Have you ever earned an achievement that didn't register?

What I mean by the title of this thread is: have you ever, to the best of your knowledge, met the requirements for an achievement and not actually received the achievement for doing so?

If so, what game, and what achievement?

It happened to me on a few games, but I only remember two at the moment:

Two Worlds - riding a horse (eventually I received the achievement, but after several rides)

Lego Star Wars The Original Triology - echo base undefeated (I was going through each level to get the "undefeated" ones and all worked that I accomplished except "escape from echo base". I ended up beating the level twice without dying and never receiving the achievement).

I'm sure I am not the only one who has experienced this.

Rainbow 6 Vegas - spec ops achievement was ballsed up for ages

Mass Effect - ally achievements not registered, nor some tech/biotic ones

Sensible Soccer - win by 10 goals and score 10 goals had to do them both twice

thats all I can think of off the top of my head, Im sure theres more

Completionist- complete the majority of mass effect

krogan and garrus ally- complete majority of the game with them

I was pretty mad when I didn't get the acheivment, but hey, at least I have something to do next playthrough:)

Just wondering, but did you do the side quests? Because in order to get those you have to do the majority of the game including side quests.

I did most of the side quests, and they were hard to get, but at least i get more exp

In Call of Duty 4, I can't ever seem to do the knife achievement. I knifed around 10 guys in a row and never got the achievement.GodLovesDead

Uh, i didnt get the headshot one on TF2 of Orange Box until about 100 headshots, and the requirement is 5... So i guess that counts.kamikaze_pigmy

Same here. The requirement was actually 25, and on the achievement menu in the game, it said I had 26/25. :?

about that one i dont thinks its glicthed i had the problem too because its not specific. you have to get 3 knifes not in a row but but in rapid succession so if you didnt do that then thats why

Most recently bioshock for research PhD... I am damn sure I maxed all photos and I even maxed all the individual creature research but still no research phd acheivement :(dinb

Ah, I guess so. Mine weren't in rapid sucession as I was concerned with staying alive. Guess I'll try to get it again.

Yeah, on Half Life 2 there's an achievement if you kill a soldier with their own grenade. I've done this multiple times & I don't have the achievement:?

I don't really care though, I don't go after achievements.

Half Life 2, when you have to kill the snipers in city 17. I got all of them but no achievement :(

I completed the game in about 23 and a half hours. Did absolutely EVERY mission possible (including ALL the side quests, my friend) and didn't get the achievements for completing most of the game, and completing most of the game with Kaiden and Garrus. Got a second playthrough to get it though, so I hope it works this time.

geomitry wars, the one where you cant shoot for 1 minute. i lasted a minute and 20 seconds and still didnt get the achevement.

I did this and only got the Quarian Ally Achievement, but not the Krogan Ally. And I used both Wrex and Tali throughout the whole game.

NHL 08 - score a goal with the goalie.

I tried this like 5 times and did it and it never registered so I just gave up.
