Super Ghosts n Goblins is an often overlooked one. I wouldn't say it is the hardest game of all time, though.
Double Dragon III on nes is up there.
...a lot of nes games aren't as difficult as people make them out to be. In comparison to today, yes, but if you grew up at that time the gameplay is almost second nature.
Teenage Mutant Ninja Turtles on NES.... Game was freaking tough! Or Bayou Billy!
Both of these are brutal. I was able to get through Bayou Billy, but TMNT (not arcade) was tough as nails... those fucking swimming sections. I'm not sure if I beat the game.
Street Fighter 2010 was probably the toughest NES game I beat.. made the Ninja Gaiden games feel like something for toddlers.
Legacy of the Wizard was killer too.

Yeah, some games do have an inflated reputation. People always bring up the NES Ninja Gaidens like they're crazy hard, but NG3 was the only one that was legit difficult.. I mean shit, I beat NG2 the same day I got it and I was only 9. The only mind numbing aspect of NG1 was getting sent back a level if you lose to the last boss.
Double Dragon 3 didn't bug me too much until the last level, Egypt. Some of those jumps were just stupid, especially if you were playing as the ninja.
Double Dragon 1 kicked my ass though.
... and Wrath of the Black Manta. I remember that game giving me some trouble, but I haven't touched it in over 20 years. Need to give it a spin and see if it's really a challenge or if I was just a noob.
Ghosts n Goblins , for me . There are some people who say Contra is also difficult but I had no problem beating it when I was a kid back then . BattleToads on other hand ... punishingly difficult , followed by old Ninja Turtle game . Anyone who is saying Dark Souls , needs to play more games .

If you think any of your games even touches the ultra troll difficulty of the IWTBTG series of games, you've clearly never played them.
I can't even beat the first level in Gaiden, one of the jumps I'm pretty sure is impossible to make, I probably tried 100 times. Add to that everything about the game is designed to **** with you, hard, and it's a recipe for pain.
Spelunky hands down. That shit is even harder than Dark Souls.

Shadow of the Beast II, probably one of the hardest games ever made.
You don't get many speedruns of that, in fact I don't think there is one without cheats because it's hard not to die, you can die within the first 3 mins of the game quite easily or mess up a puzzle and have to start the whole thing from the start.
I have seen it done though, the end boss is a complete fucker.
I could beat Battletoads with a Game Genie for infinite lives. It was still challenging. Without cheating, I could get to the boss at the top of the elevator shaft. Never could beat that robot, though. The last vehicle section should have been easy, but the d-pad on my controller was worn out enough that it was just mind-numbingly difficult. Battletoads is definitely the hardest game that I have played that was fun enough for me to want to keep playing.

DQ 2 get's easy if you farm metal babbles. Cave of rhone is still punishing without a guide however, **** that place.
Goemon 64 (was this game like hmmmmmmm infinite?) and Shadowman 64. I dunno, maybe they were hard for me cuz i was like 13 but i remember getting destroyed in these two. Goemon was the longest game i have ever played
Anyone putting anything from the last 3 gens is flat out wrong.
Games from way back (Tomb of Drewan springs to mind) where you had no safety net of a save game, the game was big and hard as **** and I never came close to completing it, despite hours upon hours of trying, we're truely hard. Other games like dungeon hack or something like that spring to mind.
Honestly, even the toughest, games these days are a piece of piss in comparison, now they are more a test of endurance.
EDIT I realise you're not wrong if you haven't played earlier games, you just don't know how hard they could be :P

For some of us, it has been decades since we've played those games, and we were children when we did so. It's hard to have perspective on memories that old, and that early in our formative years.
I'll say Dark Souls because I've played it recently (so it sticks out in my memory) and I think I'm a better gamer having 25+ years of additional gaming experience over my NES days.

You will find that you need those NES skills and that they have sadly left you after so many years of "soft" games.
My vote goes to "Bayou Billy." I never did beat that game.
Castlevania. That hallway before the fight with Death… Frankenstein and Igor… making it through all of the first phase of Dracula without taking any damage because it pretty much automatically meant losing to the second phase… it was a tough game. Beat it on Wii a few years ago via the VC, finally, but only thanks to having my progress saved. Would love to track down a working NES and a copy and do it the old school way.

I'll never forget that. :P Maybe I just sucked back then lol. But, yeah I loved playing those old games, still are fun, even today.
