I haven't seen any. I've seed game updates. Some games will go to server and download extra content that didn't make it on the disk originally. Firmware updates are downloaded from servers. Only disks I've seen with firmware updates are PSP, PSVita & 3DS. The game won't even run unless you load the firmware update.

If you haven't connected to Xbox Live in a while, your console software might need updating. If a game disc includes a newer version of the console software than you currently have, the game disc will prompt you to install the update when you start the game.

There's also the option to install from USB for the 30million+ 360s not connected to live.
Also, not being able to play Blu-rays or DVDs without an update is a blatant mandatory requirement for an internet connection - this can't be spun any other way.
