I would say Grand Theft Auto takes the cake. Although its main subject is overall crime, the way it depicts the drug world in a humorous but also intelligent way is why for me GTA is the king of storytelling for action games.

Probably missing something obvious here, but legit can't think of a single good game based on drugs.
Even something like Disco Elysium which can heavily involve drugs. Probably the best example of actual role playing in a video game, but still not that good of a game.
I appreciate how over the top stupid NARC is, but been forever since I've played it and honestly doubt it's any good.

Was there a game that GameSpot review more than a year ago that was some sort of marijuana simulator? Even though I never understand why the fuss over a mind-altering drug like marijuana that justifies its legalization for recreational use, I was surprised that game themed on the drug existed.

An old drug game most people have never heard of:
Exile / XZR
It was an action RPG for the PC Engine and Sega Mega Drive. It was like a proto-Assassins-Creed, about a time-travelling Assassin from medieval Syria.
Unlike potions in most RPGs, you instead take drugs for healing and stats, e.g. hashish, cocaine, opium, LSD and marijuana. Although the drug references were toned down in Western versions outside Japan.
The word "Assassin" apparently meant "hashish addict" originally in Arabic ("Hashishin"). Exile took this meaning quite literally by making its Assassin hero a druggie.

Afterparty, I think the drink cocktails characteristics of altering the players choices in how it affects them was a fun approach to its design.
I think it's interesting how series like Fallout treat drugs, you get short term stat boosts but suffer stat penalties after.
Far Cry 4's mind altered sections were quite amusing, either the missions where they player was drugged by those two guys, or those spiritual visions or whatever that were triggered in various sections.

I think the only games I've played with drug themes are Hotline Miami, Bioshock 1 and Deus Ex, so probably Hotline Miami
Another vote for Hotline Miami for me. Alternatively, GTA Vice City also for the 80's cocaine Scarface type story.
