Chik-Fil-A and Steak 'n' Shake (though I really do not call SNS fast food) are the best ones out there with Zaxby's not far behind. At least the first two do not server reheated in the microwave hamburgers.
I'm not sure if Subway qualifies as fast food because you have to stand there and watch them make it but I chose that because I don't feel like I need internal napkins for all the grease after eating it.
Hardee's for burgers. Jersey Mike's for sub sandwiches (subway and blimpie, btw, are terrible; almost no meat on their sandwiches) Zaxby's for chicken Steak 'n' Shake if you REALLY want fast food but it's 4 am and everything else is closed.
Taco Bell...I used to stop there on my way home from work a lot of the time...but since I've moved to England and there are none :( I've made sure I go there when I go back to the State for a visit lol.
So over here I make do with KFC or an occasional Subway.
My favourite by the way is subway as it's healthy and taste's great. I havent eaten anythingfrom macdonalds for 4 years as it is very bad for you.

Keep in mind that what you choose to eat at those places has more bearing on whether or not your meal is healthy than does the establishment. Subway has many offerings that are just as bad for you as many of the foods McDonalds offers, but you can eat healthily at both places if you order healthy foods.

I think the reason why it's hard to group Subway with other fastfood places is that it's considered to be far healthier.
Or it could be that serving one customer at a time isn't as fast as other fast food places... when it's busy. This is why I have my stomach on a screwed up hunger timetable on purpose.

Every so often I'll get a craving for some McDonalds. I can't eat much though because it makes me feel terrible and bloats me for days. My favorite currently is the A&W Sirloin Uncleburger with cheese. If you want a basic burger that's not skimpy, that one is my favorite (for what we have around here anyway, which isn't a lot).
