Ok, most of my games, if someone is talking during gameplay, I can only hear what they are saying if I am looking a certain direction in the game. Like sometimes I have to be directly facing the person that is talking to hear what is being said. Sometimes I have to face the opposite direction to hear what is being said. Facing any other way will cause the audio to go quiet. This also sometimes happens with sound effects as well. This is most prominant in GTA games, RDR, and COD.

I will check the cables ................ and I ALWAYS use headphones, its pretty much the norm for me to use them.

Sounds to me you're using a 5.1 surround setting with headphones that do not support it. Check your XBOX audio output settings and set them to Stereo.
