The pain never goes away and bugs the crap out of you. As for toothpick jammed into toenail, I can take that hit. The pain goes away pretty fast. I've had a knife stab into my fingernail.

I saw some dude get a sternal bone marrow biopsy. They basically took a thing that looked like a corkscrew and drilled it into his chest/sternum. I thought they usually take it from the hip, but this dude got it in the chest and unsedated.

Thankfully, it's an outdated practice and a simple urine sample is more than sufficient.

Actually, they still need to swab the urethra if they are checking a male for an STD. They take both a urine culture and a swab. The swabs are smaller and less irritating, but they still do it.
Realistically like day to day stuff would begetting a paper cut somewhere like a finger or something or just even thinking about one always makes me cringe but one of the most painful things I can think of would be a paper cut to the eye or something god could you imagine ?

Teaching a new guy how to cut take offs for duct work and explaining to him not to stop and cut stop and cut because it creates "hang nails" basically long razor sharp points of metal and him not listening and stabbing one through his hand when the cutters slipped.
