Resident Evil 4 by far, with Silent Hill 2 as a runner up. I usually don't get scared by a game, but there were quite a few times in that game that made me jump or just plain gave me the willies. Examples: (SPOILERS AHEAD!)

 -Those regenerators. Man they were scary. Plus the fact that you had to nail the parasites to kill it, and the spikers had them on their back which made killing them far more difficult.
-The blind guys. Not only was it heart pumping having them walk right by you breathing heavy and all, but trying to run away and hearing the "Shiiiiiiing!" of their blades unsheathing and then their rapidly approaching footsteps getting louder and louder was downright freaky.
I think a lot of the tension from that game came from the "right behind the back" perspective and incredible sound design that left you wondering what was going on behind Leon.
EDIT: Thetraitor, I enjoy your avatar, but am curious as to how GS let that pass? Cool nevertheless!
Resident Evil was the scariest game I have ever played when I was little. I got it for the DS last year and I don't know how a game like that could have scared me so much. RE is such a cheez fest :)
Most recently was Condemnedl: Criminal Origin. Being alone in the dark, hearing spooky sounds, your mind plays tricks on you!!

I found the Marine campaign kept my nerves up. Not so much the Alien and Predator campaigns though. Although not scary today, there were games that did use to scare me. Advanced Dungeons and Dragons on the Intellivision. Alone and the Dark and Doom on the PC. When I played those late at night, lights off, it creeped me out.Probably Silent Hill 2.
Most of the RE games are good for some shocks here and there, but I think the SH titles are consistently scarier throughout.Â
It's gotta be a toss up between Clock Tower, and Fatal Frame (Project Zero) 1 and 2.
Those games scared the absolute crap outta me, i still can't sleep sometimes.
I don't really play many scary games, so mine may not be as scary to some...
Call of Cthulhu: Dark Corners of the Earth: I'm not even close to being done, but boy does this game get my heart pounding. The opening build-up was pretty creepy...but the hotel sequence was terrifying, and the game does not slow down at all from that point. The fact that you have no weapons makes the game about a million times scarier than others I have played, because you know that no matter what you do, you cannot let your enemies find you. So far, it feels like one long, intense chase and it barely lets up at all. Can't wait to finally get a gun and put a bullet in these crazy people's heads....pretty sweet game.
Also, another game that sort of scared me was Indigo Prophecy, SPECIFICALLY the first-person archives sequence.
Maybe first-person games with creepy music just freak me out in general, who knows.

Indigo Prophecy? :| The only thing scary about that was pushing an old women in a wheelchair through an abandoned mansion.
No games have really scared me but a few creeped me out. Haunting Ground is an example of a creepy, disturbing game.
Also, there were a couple of moments in Resident Evil 4 that genuinely made me jump out of my seat or made my heart beat like the part my head got grind by the chainsaw or when Salazar's mutated form ate me.

Silent Hill is scary, but Fatal Frame actually gave me reccuring nightmares. :)
