There were certain caves in Morrowind that were unnerving to explore, along with everytime I came across a vampire.
Speaking of vampires, in Vampire the Masquerade: Bloodlines, the haunted hotel level was a pain to get through the first time I played it. The rest of that game was pretty tame, but damn it I hated that part.
Not really scary but it was very chilling when I was doing that one quest in Oblivion where I had to discover the traitor of the Dark Brotherhood and when I found it inside the lighthouse....eugh.
Also, that music when I'm playing the Sims 3 and the Repo man comes to my house always gets to me; might not be as bad as the music in the Sims when your house is being robbed but it still gives me a jump.
I'm not sure if Hitman: Contracts is non-scary enough for this list, but the psychotic butcher in the Meat King mission was fvcking terrifying.
In Uncharted, it's those chapters w/ those 'things'.......the darkness, plus the total isolation in those instances creep the s*** out me b/c I simply was NOT expecting it. *shivers*
Damn, I so glad when I got out of that part, lol.
I was just gonna post Hitman: Contracts! Not that, though. I was thinking the ghost part.

Yeah, that part did freak me out the first time I saw it.
Thief: Deadly Shadows in the orphanage. Real creepy vibe and for the first 20 minutes it's like Amnesia; you're exploring the area and you always get the feeling something's watching you, which was pretty unexpected considering you're usually the hunter.
STALKER: SoCin the labs was very unsettling and you just want to rush through the place to get it over with as it's so tense. It's harder to pull off this feeling in a game where you're armed to the teeth so this took me by surprise.
Minecraftgot me paranoid a few times in the tunnels and always makes me jump when a creeper comes at you out of no where.
Speaking of RDR, I had a kind of Survival Horror kind of moment.
I was doing the 'knife a goddamn bear' challenge in the woods, and I was on this bear's tail for a while. The bear ran around a corner, so I stood where I was, just waiting, and all of a sudden 3 bears come sprinting at me, it was terrifying hahaha. So, naturally, I got the hell out of there.
A giant blob that fell out of nowhere in Zelda II: Adventure of Link. That thing scared me when I first saw it in the final palace.
My first encounter with the Flood in the original Halo was pretty creepy. Even though I was playing Halo 1 years after its release and knew that the Flood were in the game, I was still a little nervous in that whole level.

The first crocodile encounter I had involved me innocently picking a lotus off a river.

For me it was worse lol, changed my mood. I saw this deer walking by near the lake, and he just fell in ( and drowned). And I was like "LOL dumb dear, dumb AI", so I walked to it to skin it, and out of no where, a croc pulls me in. Lol, I was just thinking the same shvt reading the title. Those chapters gave me a Resident Evil nostalgia not knowing wtf was waiting around the corner! Like you, I couldn't wait to make it off that boat! I love intense, unexpected surprises like that in games.
In Minecraft, exploring dark caves and hearing creeper and zombie sounds...
And in Half Life 2, the head crabs....
In Minecraft ambient noises. I am not scared of dying in MC even though I like to keep atleast a stack of diamonds with me for some awkward reason. :P.
Caves and dungeons in every single game also scare the hell out of me. this includes MC. (The reason I always dig a quarry instead of caving.)
Generally, games that had large bodies of water and wierd underwater creatures would always creep me out. I'm facinated and scared of the deep ocean. One thing that comes to mind is Mario 64 and Jolly Roger Bay. That giant eel made me uneasy as a kid.
Another from the 64 bit era was Shadows of the Empire, the Sewer levels later in the game. The surface of the water wasn't translucent, so you could never tell the depth of the sewer water until you jumped in, and you would find out about the dianogas, including the main boss of the level being a much larger version of these things.
Yeah, I can't stand weird water creatures.
The Thief games usually have great atmospheric haunted levels, especially the Shalebridge Cradle in Deadly Shadows. Anyone who has played it can vouch for how freaky it is. There are also scary moments in the game where you're like "Sh!t sht!t SH!T! RRUUUUNNNNN!"
Also the Bottom of the well and Shadow Temple in OoT were pretty creepy, along with the entire area of Ikana Canyon in Majora's Mask. Both are great examples of kids games that are downright freaky. There's also somethingunnerving about the first tiem you get to the Fiendlord's Kepp in Chrono Trigger... all the kids, the compeltesilence... very chilling the first time around.
Uncharted: Drakes Fortune - Last few levels with the surprise twist of mutant creatures was really nerve-wracking on first playthrough.
Far Cry 3 - Shark and crocodile attacks occasionally made me jump when unaware.
Batman: Arkham Asylum - Killer Croc's lair was quite nervy too.
SA-X attacks in Metroid Fusion. (hmm, why did that hatch just op- oh crap oh crap oh crap).
Speaking of Metroid, "lights out" in the Phendrana Drifts from Prime. Those who've played it will know. Cant' see sh** without the Thermal Visor.
Forest Temple and Ikana Canyon were always unsettling as a kid. Creepy music in those areas certainly did'nt help.
And I'm not even going to lie, I have jumped on probably more than one occasion thanks to ninja cougars in RDR.
I would say Knight Contract. From start to finish. It still gives me the chill, awful game. Worst thing is I finished it :P
