Resident Evil 3 Nemesis, he would always pop up when you are low on health where Jill is dragging her leg and then there is a panic to make it to the nearest exit.
It's dark out, I'm playing RE4, I'm in the castle, low on ammo, and I keep hearing people whisper, "coohelo coohelo" or whatever and I cannot seem to find where the sound is coming from. Torture. Doing SMG2's final daredevil level was a little scary too. Just trying not to get hit. :P
Pretty much all of SH3, it was one of the few games where I really felt helpless in the face of danger. Beathing down a boss with a crowbar and no health packs was rather intimidating if not satisfying.
The only other moment that comes to mind is in Call of Cthulu: Dark Corners of the Earth when you have to escape your hotel room while locking the doors behind you. I think it is the most suspenseful moment I've ever experienced in a game (if not the most, then it's very close to it).
First time running into that creepy girl from Fear & the broken neck ghost from Fatal Frame. My husband was watching me play both times and he jumped higher then me. Good times! :)
batman AA [spoiler] scarecrow part thought my copy of the game was faulty and restarted my ps3 3 times before checking online to find out its suppose to happen [/spoiler]
