In Valhalla I went 25-1. I was close to completing the match without a death, however an enemy sniper noticed where I was shooting from and took me out. Seriously, I have yet to get a single perfection and I am Rank 78 xD.
I used all of the codes given to me from my Limited Edition copy of Halo 4 o -o... I'm sure you can find one for sale from eBay.
