In general, yesterday with OoT 3D. For PC...probably a few months ago. Now that I have faster internet, DD is starting to become the norm for me.
Just last Thursday when I bought Portal 2. Friend works at Target and it was 30 bucks, but ended up being cheaper because of his discount. I never really touched the disc because I activated it on Steam, but I at least bought the physical copy.

The other day when I bought The Simpsons Season 13 DVD set.
Oooops game.....
That'll be the Empire: Total War and Napoleon: Total War combo.

I like that combo. Install with the disc. Then activate (via Steam or something else) and never have to use the physical media again.

Didn't even do that. Went into the corner, chose Activate a Product, entered the CD key, and it downloaded. I download at about 7.5mb on Steam, so it's just about as fast as installing it. I was already getting impatient downloading Lara Croft and the Guardian of Light and it's less then 3gb. Heaven forbid if I have to download X-Plane 9 which can total 7 DVD discs if you include scenery for the whole world.
CoD Black Ops just because I wanted the case for some reason, although the game had some issues at launch, aside from the fact that MW2 is so much better that it seems like the games were developed on different planets, and the one BLOPS is from, sucks while all the good looking women and alcohol are on planet MW2, but I digress... Prior to that, I bought The Orange Box on launch, although just yesterday I ordered a physical copy of RFA for a friend as a gift since it costs less than half the price of the digital download on Amazon which is really, really odd.
I'm buying a physical copy of Deus Ex Revolutions because it has cover art that is literally worth more than some premium game packages are worth... It's the most amazing cover art I've seen in a long time, and it does help that the game itself is probably the best single player experience of its kind(meaning what it does, it does very well).
Last physical game was LA Noire on PS3 in May, last physical game on PC was Starcraft and Diablo 2 in March
Fallout: New Vegas (whenever it was released) but, other than that, everything has been a download form one on-line service or another.
It has been almost 3 years since I bought a game on physical disc. That was Wrath of the Lich King in November 2008. I pretty much only played WoW then so I didn't buy many games. Once I quit WoW, I found out about Steam and bought exclusively from there. This year I started branching out a little bit to other digital download sites now that Steam has some good sales competition. I don't care about collector's edition stuff or having a physical game manual. Those things just collect dust after the first week.
It's been a while. I think Portal 2 was my last physical copy purchase from Amazon.com because it was only $29.99 only a week from launch day.
Deus Ex: Human Revolution will be my next hard copy purchase, also will be from Amazon.com.

same here, i also picked up portal 2 on amazon for the same price before it launched tho. that was my last physical media. then before that, i think starcraft 2. are we talking pc wise only? cuz when it comes to consoles, i think gt5 was the last game i bought.

Starcraft 2 haha yup. Blizzard still makes them with the cardboard material haha. Love it, vintage, classic.I think Half Life 2 was the last boxed copy game I've purchased. Or I might have rebought Diablo 2 sometime afterwards actually..
I'll buy a physical copy of game if I can play it without a disk or download it if my disk stops working for some reason (I always take care of my disks, but sometime they just break lol). For that reason I pretty much only buy Blizzard games in physical copies. I'll probably get Mass Effect 3 CE in a physical copy if it comes with something worth buying in a boxed copy. Does anyone know if you need to a disk to play ME2, or if you will for any upcoming or previous EA titles(with origin, I'm guessing you'll be able to register you boxed games there and have them available for download too)?
I just brought Dead Rising 2 last week and I brought Fallout NV a couple months ago. I've also preordered BF3 and Skyrim which are both physical. I like that these are mine and I'm oldschool, I like my cases!
im thinking about getting swat 4 soon, and im going with physical because it is cheaper. D2D still have it for $19.95....
i got hitman contracts recently, and also splinter cell pandora tomorrow. but that was because i couldn't find DD copies. :)
Anything I can buy that isn't Steam related I'll buy from Amazon, Newegg, or Best Buy. And even some Steam games like Portal 2. Basically I like having the choice to install games on another HDD and Steam doesn't like that so if the game doesn't require Steam I'll get the hard copy. Also I like to hand my games down to my son and it's easier if their not Steam related.
That said I did get a bunch of games on the Steam sale this summer so I'm not against DD's. But in answer to your question it was either Witcher 2 or Portal 2... can't remember which I bought first.
DD is becoming the norm for me too, but if brick and mortar offers a better deal, then I'll go there. Last game I bought a physical copy of was Alice 2: Madness Returns.
Well probably for the GameCube (picked one up not too long ago) Was more than likely the LOZ collectors edition.
Since we plan on getting a 3ds in the future, along with the WiiU, and a PS3. We probably will have a lot of games on physical media.
But when it comes to pc... We haven't bought a physical copy in some time, and don't plan too. Maybe The Old Republic for sheer size of the game, but I much prefer digital copies. (Uses considerably less resources, and don't have the burden of the disc, and not to mention all these wonderful Digital sales througout the year.)
