What would leave me slack-jawed?
Presenting... ...The Legend of Zelda Wii.
Supports both awesome motion controls and awesome Classic Controls.
More open world than previous Zelda games.
More populated.
Starring the voice of Gary Oldsman as Ganon, and Gwynneth Paltrow as the titular Zelda.
For sale. In Stores. And here in E3. Now.
A new console! One that is a powerhouse and HD. I'm sorry but I just can't help it, I really really need this from Nintendo soon. It would leave both srprised and painfully excited!
On the software front a new Starfox would just be the cat's pajamas. And if by chance it was done by retro?....Oh man I might just die right there.
A new zelda game that will be launching alongside the new nintendo system that is scheduled for release by christmas of this year.
Anything else is meh worthy.
Presenting at least two games that might actually be good.
Not announcing any new gimmick controllers.
I'd be like: :o
- A Zelda Wii which reinvents the series in terms of gameplay, while maintaining the familiar atmosphere.
- A First-Person Shooter developed by Retro Studios that has both a lengthy online campaign as a very deep online multiplayer mode that supports WiiSpeak
- A new Paper Mario or Mario RPG for the Wii that makes the series more RPG-oriented again, but maintains the option to play as different characters like in Super Paper Mario.
- Star Fox Wii that isn't just a rehash of Lylat Wars, but has a more adventure game-like progression (as opposed to the more arcade-styled LW and Assault), with awesome motion controls and a great mix of on-foot and Arwing missions.
Give me that, and I'll faint.

Well, they did that last E3.
It think i would be surprised with a Star Fox game made by Factor 5 where the action will be finally back to tha air and not with lame foot sections.
Also, an MMORPG Pokemon would be incredible cool.
Finally, a new console Kirby game, it may not be surpris or shock me that much, but i would be happy as hell.
Call me crazy, but if it's an announcement so surprising that I'd be at a loss for words, I don't think it would be something that I can- you know- actually talk about now. :P
Therefore my answer is:
Something TRULY unexpected.

After thinking some more, I'd also like to add:
- A sequel to Deadly Creatures. The first game was great, but there was a lot of room for improvement, so I'm hoping to see a new title. The possibilities for a new setting would be endless (the Amazon, a forest, hell, even underwater)
- F-Zero Wii WITHOUT motion controls.
- Pikmin Trilogy (awww yeah).
- A new Rabbids platformer.
Finding out that backing up the Eternal Darkness and Luigis Mansion trademarks a few months ago actually had a good reason (sequels) :(
If Nintendo reveals a holographic storage add-on and a firmware update that will allow the wii to read HVDs, and than it is revealed that Zelda wii will be on an HVD and will launch this add-on and reveal Zelda wii will take up roughly 5TB and make the graphics of Crysis 2 look bad in comparison, I will be speechless. Anything less will be an utter disappointment.
MOTHER 4 to be released on Wii in a cel-shaded hand-drawn art-style along with the first three MOTHER games localized in one DS game.

A full fledged 3d pokemon game would probably be the biggest surprise for me. I'm not a huge pokemon fan, but I still can't believe it hasn't happened yet.
I guess another game would probably be a Nintendo title seriously aimed at the adult crowd filled with gore and blood. That would probably be a shocker for everyone. Especially if it had high production values
