It can run into any definition of hard core. Mine is Might and Magic 6 which I have yet to beat to this day sinking several dozen, even hundreds of hours into that game. It completly destroys any RPG experience (next to KOTOR for story reasons) due to it's massive complexity of D&D style rules. It also has a perfect difficulty slope, experience slope, gold slope, and loot slope, as there is always something more stronger than you, a common enemy that becomes a natural border of "I can't go there." without feeling artificial like "the path opened!".
It may not look like it, but these guys are VERY hard even for someone who sunk one or two hundred hours into the game, and that's not even the tip of the iceberg
Same with XP in that leveling comes very naturally and at a pace that does not seem to quick or to slow, and there is something you always want to upgrade more of. The gold comes into factor that even in high levels, the shop offerings still have something to bear, and reviving a dead comrade would break ones bank sometimes. Loot also comes in the same way as there is always something better to be found. The graphics may seem primitive to todays standards but actually work into it's favour as it makes the monsters more scary if you could belive that.
That flying eye still gives me nightmares....
What's the most "hardcore" game that comes to the forefront of your mind?
