What medieval roleplay PC games can I play?

I'm searching for PC games in a mainly medieval, fantasy or supernatural genre. I like open world roleplaying/simulation games like Elder Scrolls: Oblivion, which my PC can run quite well with mods with only slight lag even on Ultra High settings. I'm not after games like Stronghold or Total War, I want to be immersed into the game as one person not controlling the world from above.

Even though I'm mostly after medieval games, I'd be open to other games except from modern warfare and shooter games ( I have too much of that stuff ), zombie/nuclear apocalypse and futuristic, space-setting games.

Bonuses:

Mounts or ability to shape-shift into natural and/or supernatural/fantasy creatures, i.e. dragons, werewolves, etc.

Customisable character or choice of characters.

Lots of content - weapons, side quests, armour, items, etc.

Reasonably good graphics - no pixels.

Good storyline.

Modding community.

My computer specifications:

XP Professional Service Pack 3

Pentium 4 CPU 2.8GHz ( Single core I think. )

3.15 GHz, 2.00 GB of RAM

GC: ATI Raedon 9800 Pro Series

I am aware that my computer is a dinosaur, which is why I'm struggling to find some good PC games, but getting a whole new computer isn't an option for me at the moment. I'd be grateful for any help,

CRPGs have been dead for a while but there has been somewhat of resurgence lately.

Still in early access but you might want to give it a try.

Pillars of Eternity will be coming out in 2 weeks. Should put that on your list it will be like old-school D&D RPGs such as BG, IWD, and NWN.

I've had a look at some and none have jumped out at me yet, but I'll keep delving.

Kingdom Come looks exactly like the sort of thing I'm after, only I don't know if my computer could handle it.

Well, I would like to help you but there's only a slight problem here

That pc can't run oblivion at ultra high settings. Heck , it can't even run oblvion at low settings, not when you go outside anyway. Are you sure you're not talking about the elders crolls: morrowwind. If that's not the case you must have mixed up the specs. Can you go to hardware specs (you know right mouse button on my computer, properties) and take a screenshot (press prtscr button, and paste it in paint, then upload picture here)

If you can run oblvion at ultra high settings, you can pretty much run any game.

Sacred Gold won't install on anything later than Windows XP although the GoG version might. Another oldie I like is Vampire: The Masquerade - Redemption.

I can play Oblivion ( yes, definitely Elder Scrolls: Oblivion ) on Ultra High, despite it having a bit of lag here and there, so I usually only play it on Medium or High. The specs are quoted from what is showing up on My Computer and from what I know is inside the computer. No mistakes.

I'll get a screenshot up soonish.
