Well, Oblivion has some good sandbox design imo, although the RPG elements aren't the best. Then there's Two Worlds 2, which I haven't played, but it's an open world RPG.
Fallout 3/NV are open world RPGs with FPS elements, and the STALKER series are open world/sandbox FPS games with RPG elements. Especially Call of Pripyat has excellent sandbox design.

Well i already have Oblivion, Fallout 3 And NV but im gonna check out Two Worlds 2 and STALKER. My friend has STALKER and he says it's good so im gonna check it out!
Gothic 2 (the game is pretty old though), Gothic 3, Risen and Arcania are some good examples. I never played Risen or Arcania myself but I heard Risen is pretty good while Arcania not so much. If you have a good computer I suggest you grab Arcania if not for anything else for superb graphics.
