I don't think Carolyn has accepted the Joker's death at all. She's seems like she's more in denial of it when Rocksteady has confirmed, a thousand times already, that the Joker is DEAD, at least in the Arkham universe. So I think she needs to get over that. I'm sure that there's going to be flashbacks or a connection with him, due to Harley's presence, but it's not like Rocksteady hired Mark Hamil in the background, had him do some Joker lines, and then waited until the game to reveal him again. He's dead, dead, dead, dead, dead.
@Metamania I agree with you totally on that, Rocksteady's Sefton Hill said in an interview that "The Joker" Will not be in "Arkham Knight" and he also stated just like you did, that "The Joker" is Dead (he actually said it like 5 Times in a row! so people would get it through their stubborn Brains! :) like this person in the video who has no idea what he-she is talking about. but I do think Batman will experience some kind of crazy flashbacks that has to do with "The Joker" being in his head. most likely he'll be drugged up with Scarecrow's Fear Gas, and he'll have to overcome a few obstacle courses like he did in "Arkham Asylum". with joker's death screwing with his mind and than some. that's what I expect for any Joker cameo to happen in this game, since he's.. well dead. but i'm sure it will an EPIC experience! and since this is the last Arkham game from Rocksteady, I'm sure they would expect us to want to see The Dark Knight's greatest enemy for last time! :) cant wait!
