In my first playthrough I explored quite a bit and found a bunch of them. After I completed the game I checked the complete list of diaries, and turns out I was missing 7. The same guide also contained the location of the diaries. In my second playthrough I just kepts an eye out for that and other few achievements I was missing.
I got all of them in my first playthrough. They're all named so it's not too hard to keep track of them all, just search and check online for the list for any you're missing and they'll give you the final locations.
