No game ever made is worth 10/10 if you are assuming 10/10 is a perfect score meaning there are either no problems at all with the game, or the game went above and beyond with all other features that makes up for the things wrong with it and then some, I don't think a game has ever done that unless you are talking about stupidly simple games.
People need to break their reviews down into different aspects of a game, for example Graphics, Gameplay, Story (depending on the game, exchange for lore or atmosphere or realism, or count all of them together), Modability & customisation (if applicable, big feature of RPG games mostly), features vs bugs then add all those values up to a score out of 40 or 50. Thats the only fair way of reviewing a game. One 8/10 may not be equal to another 8/10, one game might have shit graphics but good features, another might have poor features but good graphics.
Reviewers these days just seem to pick a number out of their arsehole or a number that creates controversy. I want to hear valid criticisms, I don't care what score people give a game as long as they write down why they gave it that score and not just say it's great... that's not helpful.
The closest I would give to 10/10 is GTAV, and I would only give that a 8.8-9.0 out of 10, because I can appreciate what they had to do to even create that game on last gen consoles and it rivals next gen games on scope and features and even graphics, the story was quite funny as well for an open world game, and there was quite a bit of customisation you could do to cars and to your characters, the features way outweigh any bugs the game had. The only points I would mark it down on are driving physics and disappointment of the way heists were advertised/implied.
Last of Us I would give a 8.3-8.5/10, it's story was spot on, the graphics are the best you are going to get out of a last gen console you can see the attention to detail and optimisation, the gameplay was good for what it is perhaps not the best ever but good enough and did have some uniqueness to it, you can't really mark it down for customisation not that sort of game, and there were not all that many bugs worth noting..
Dragon's Dogma: Dark Arisen I would give a 8.1/10, it's story although not great was more than adequate the atmosphere is great, there was some lore in mix as well. The game and the graphics were good for an open world game especially the lighting and shadows, the gameplay took a bit of getting used to but it was one of the best combat orientated RPGs I've ever played and had some learning curve which is a good thing although was not fun at first but there was a moment when it just clicked, there was quite a bit of customisation especially with the pawn and dual skill system and variety and rarity of equipment, plenty of bugs mostly outweighed by features some annoying things where quests could not be completed or failed without warning, frame rate could have been better, should have been released on PC.
Dark Souls 2: I would give 8/10, story non existent however the lore and atmosphere more than make up for it, Graphics are more than adequate even if it was downgraded, customisation is OK not the best it's really just a dodge vs tank thing going on with armour and I don't like the way they force you to gimp your character in both looks and stats in order to get bonus effects like +souls or +loot feels restrictive feels like you are gimping your character or wasting time if you don't wear it, would be better if they just removed those items entirely, the weapons are very good with good variety combined with skill sets. Gameplay is the one thing that sets this game from a lot of games, steep learning curve unforgiving but very rewarding, such a unique experience. Not many bugs to speak of well you have initial PC release WTF was that but has been fixed..
There might be a few more games but those are the ones that come to mind when i think of good games that you should not miss. Not played anything on Xbox to comment on those exclusives.

Assassins Creed 4 (yup loved it more than AC2.) 7.5/10

hard to give a game a perfect score, i used to like the decimal point system for scoring games. i thought metal gear 4 deserved it at first but it only received that due to hype and it being the ps3 first good game, metal gear 3 was better and still is.id give it a 9 where part 3 would have been a 9.7 . red dead was great a solid 9.5 . im going to get flamed hard but The Last of Us deserved the 8 it got its a crouching simulator. i wouldnt give any of them a perfect ten not even the souls games.
Yeah I'm too easy a critic probably. Still, these games were all amazing and even if they weren't revolutionary, they still contributed greatly to either the genres as a whole or to their respective franchises.

@sailor232: Agreed. Portal 2 was the closest to perfect to me. I suppose if you wanted to nitpick in places you can, but guess what? Any game can be nitpicked.
Single player was amazing but then you've also got the online single player and multiplayer user made maps that will keep the game going forever. Playing the game co-op with a friend on user made maps is the best multiplayer experience out there.
@edwardecl: If you want to hear valid criticism then doing away with a score altogether is the best system, not trying to find an empirical numbering scale. You don't see academics tacking on a score to the end of their tenure papers, academic journals, or critical analyses.
@yokofox33: Really Valkyria Cronicles didn't make you question it's greatness? Don't get me wrong it was a good game, but the slow pace of battles, having to wait and watch your opponent move with no option to fast forward was very tedious.
I only gave 10/10 at 2 games: SotC and RE4
so to answer to TC question: None. Some came close but they werent quite there yet: Journey, RDR, Dark Souls, TLOU and GTA V

and haters said no one would regard U2 as a classic in the future. This just proves they were wrong.
btw I actually find U3 better than U2 and I liked the new TR better than any Uncharted but dayum, it feels good when haters are proved dead wrong

DotA 2 springs to mind but if you consider the community to be part of the game then it certainly isn't a 10

If I didn't post on a gaming forum, I probably wouldn't remember those two games either. I only remember Lair because it's one of the infamous flops in this forum's history.
None, because to me a 10/10 is a perfect game, and I've yet to see one. Maybe the original Starcraft, but that wasn't last generation anyway.
The Orange Box, Mario Galaxy, Mass Effect 2, Arkham Asylum, Uncharted 2, Portal 2, and Bioshock were about the only close to perfect games I can remember from last gen. All were 9.5's in my book, but had minor flaws that kept them from being 10's.
After all these years I thought people would finally let go of the "can't be 10/10 because it's not perfect" nonsense... oh well.
It's a scoring scale guys. Obviously nothing in this world is perfect. 10/10 is just a way of saying "hey I think this thing is fucking awesome in comparison to other things like it!"
Anyways, SMG2 is the first that comes to mind.
