uncharted medals

I have a question about the medals in Uncharted. There are ones where it tells you what to do in order to get it, but then near the bottom of the list it says play the game to earn medal on like 10 of them and I don't know what that is. Can anyone help?

last four are finishing the game on easy,normal,hard,and crush difficulty.

Most of the hidden ones are kill x amount of people with y weapon. The last 4 are beating it on each dificulty, and there is one other hidden one is kill 3 people with 1 explosin 5 times.

The thing is, you have to beat the game on crushing to get the most fun reward: infinite ammo. Although, super-slo mo is pretty cool, but it does make the game incredibly easy, even easier than infinite ammo.
