It's normal, some games will have a lot while others will have none at all, and it'll be different depending on what videocard you use.
Z-fighting is usually the fault of level designers. After all they are the ones modeling the levels. Either that or its the fault of a bad LOD algorithm. GTA is a good example of this, and BF3.
To get rid of Z-fighting you should have non-overlapping polygons or maybe the engine should be coded in such a way to compensate for it. In other words... there's not a whole lot you can do about it.
