Which game was the hardest for you? And why? Are you playing this game now? Maybe you finished it and forget like a nightmare?
For me it was Ninja Gaiden on NES. It was too punishing that I haven't tried to play it for 2-3 years.
trying to get iron and a land claim on a already populated server without any help in Wurm.
why? because npc beasts breed, meaning the longer the server is up with nobody cleaning out the area, the more danger there is.
starting new without a mount, that job (was) almost impossible.

Akumu mode in Psycho Break
Master Ninja in Ninja Gaiden Black/ Sigma
All 100 levels in Superankā
And Dante Must Die in Devil May Cry 3 (PS2 original release)
just to name a few(ΦωΦ)
Gaiares for the Sega Genesis/Megadrive. And it's not even close.
Owned the game for 20 years and never made it past the third level.
Dmc 3. But I did beat the game.
Hollow Knight and guacamelee comes close as they have some of the most difficult platforming sections.
Super Hexagon. But also beat this one too.
Also I don't find the soulsborne series that hard anymore.
There is probably a lot more but I cannot remember them right now.
Cuphead is probably the most recent one that killed me. Legitimately wasn't able to finish it unfortunately.
Most difficult game I DID complete though was probably Super Meat Boy, Battletoads, or Ikaruga.
Edit: Oh Super Hexagon is rough too. Haven't gotten past Hexagonest yet.
I've played a lot of hard games over the years but out of the ones I've beaten I would go with Medal of Honor Frontline on PS2. I can remember being legitimately on edge during that game. There were no checkpoints during missions and no regenerating health. I only played on a lower difficulty (easy or normal) and even then just scraped by. I can remember feeling so relieved when I finished it.
F-Zero GX. In story mode on very hard difficulty you had to be absolutely FLAWLESS with very minimal room for error to win those races. So many raging moments I had with that game lol...
I kind of wish I could have played F-Zero GX. I've played many Nintendo games alike but I missed playing with Captain Falcon. So How have you been.
You never played F-Zero GX?! What. The. Hell?! Well, you were kinda getting laps ran around you by yours truly so I guess that can slide. Gave you enough rage moments to make up for your lack of F-Zero GX rage moments lmao.

Games of the 8-bit era were generally a lot harder than today's games, because there were no continues, no regenerating health and only few checkpoints
The hardest game I have completed without cheating is probably Ghost'n'Goblins on Commodore 64. Green Beret is even harder, but I never finished it. I only beat level 1.
I'm normally pretty competent with shooters, but Max Payne 3 gave me so much trouble back when I played it that I switched it to Easy just to get through it. Not sure if it was actually a hard game or if I was just really bad at it. Maybe some combination of both, it was a long time ago when I played it so maybe I wouldn't struggle as much now.
The final fight with Doc Ock in Spider-Man 2 had me crying literal tears of frustration. I just couldn't do it. No idea why.
Any competitive mp game. Shooters, Fighters, mobas, racing you name it.

Souls except BB. I don't think I can finish a Souls game without getting bored from trial and error builds without some community guidance.
