Surprised no one's mentioned the Joker yet. But I guess it goes without saying.
EDIT: Sylvia Christel from No More Heroes. Not in your face insane, but there's something incredibly off putting about her, at least to me.
Fawful from the Mario & Luigi games is what I consider insane.

Actually I think Sylvia is the least insane character he created. That I have seen anyway.

I wanted to and mention a lot of the others from Arkham City and Asylum but then I'd probably end up being here forever or longer than I should be.

After that ordeal, he asks everybody to adress him as the King of the Bugs. It was also revealed through his mother's messages that he had a strange apetite for insects.

Really? Her bi polarity makes her very unsettling to me. Every scene with her seemed really... off.

Yeah I guess. But I still consider the guy willing to become the number 1 assassin just to have sex with her more insane. Of course you have be pretty insane to challenge someone to do that too. But the guy who does it is more so in my opinion.

The most insane character has to be Vaas from Far Cry 3 and he was a fantastic character, most other characters in this game were dull compared to Vaas! I-AM-N00B
The issue was that Vaas had barely any screen time and swore way too much to take him seriously.

ok, this thread has gone on too long without mentioning playing as a Malkavian in Bloodlines. Hands down winner. TVs talking to you, imagined phone conversations and the ability to make other people as crazy as you are, down to even convincing them to kill themselves so you don't have to, as you brazenly turn the vampire world on its head in nothing more than a cowboy hat and some nifty ass-less chaps. NPCs being insane..meh. Playing as an insane character with full control of the dialogue? Win.
