  • Cool Spot - An often overlooked and never played game in the Genesis (and SNES's) library because it looks like a crappy licensed game. It's based off of a short-lived advertising character for 7-Up Soda. But it's actually a pretty good 2D platformer that looks impressive for the time.

  • Gargoyles - Very impressive looking (for the Genesis) action-platformer based on the Disney Cartoon.

  • The Punisher - The only home port of the Capcom beat-em up arcade game based on the Marvel Comic. Great beat-em up with lots of destructible items in the environments and weapons to use.

  • Rocket Knight Adventures - A great 2D platformer with a lot of charm and polish made by Konami. It's hard to describe without playing it. It just has a feel to it that feels right.

  • Thunder Force II - This was the first game I ever played in the Thunder Force series. It's a great game that has both traditional side-scrolling shoot-em up levels, and has these awesome top-down view shoot-em up levels that you can explore in any direction. I just love the concept.

  • Vectorman 1 & 2 - Not so hidden because of the Sega collections, but I don't see it mentioned too often and it's a fantastic run-n-gun platformer)

  • X-Men 2: The Clone Wars - Wasn't a "hidden" gem back then, but I hardly see it mentioned anymore. Easily one of the best beat-em ups I have ever played. Lots of platforming involved, fantastic graphics and sound, and a great implementation of Street Fighter-esque moves to execute special attacks. One of the reasons to own a Genesis, IMO.

More well-known Sega Genesis Favorites: Sonic the Hedgehog 1-3, Sonic & Knuckles, Golden Axe 1 & 2, Comix Zone, Castlevania: Bloodlines, Contra: Hard Corps, TMNT: Hyperstone Heist, Super Street Fighter II, Mortal Kombat 1-3, Maximum Carnage (Spider-Man).

Trouble Shooter - awesome anime style shooter with cute girls and gun

Grind Stormer - an amazing shooter with awesome powerups

Valis - cool anime style action game

El Viento - another awesome anime style action game

The Flintstones - not many people knew a Taito flintstones game was released on the Genesis, but its a really great platformer with cool bosses

Warsong - a really awesome RPG that is just starting to get recognized nearly two decades after it came out.

It looks VERY nice for a Genesis game. But the platforming felt very basic to me. So it wasn't nearly as fun to me as it was pretty.

That's understandable. You'll probably have more fun playing Sonic. But it is a very unique game and a gem nonetheless (IMO)

^ Super Thunder Blade would be a great game, if it didn't run like crap.

A few of my favorites:

  • Starflight
  • Master of Monsters (Abysmal translation, but a fun core game)
  • General Chaos
  • Mutant League Football
  • Rocket Knight Adventures
  • Toe Jam & Earl
  • Phantasy Star II & IV (Not exactly under the radar, but a necessity for the RPG fan)
  • Shining Force 1 & 2 (Same as Phantasy Star)
  • Gain Ground
  • Rampart
  • The Immortal
  • Zombies Ate My Neighbors!
  • Star Control

I'm sad no one mentioned Shadowrun. Shadowrun is in my opinion THE BEST Sega Genesis game, imagine if Fallout 3 or The Elder Scrolls Oblivion were made for a 16 bit console, that's pretty much Shadowrun in a nutshell imo
