Beatmania IIDX. I can beat most songs on Normal difficulty... but Hyper and Another (and Black in DJ Troopers) is just beyond me.
Devil May Cry 3 on Dante Must Die is also incredibly difficult.
I'd say Demon's Souls. I went straight to the first boss, killed him in a few tries, then I wasn't able to do s*** the next 2-3 hours I played, I always died in the first 15 minutes of the next quest.
I've always thought Guitar Hero was kinda hard when you get to hard (I'm usually able to do about half the songs in hard, I don't want to think about Expert). Maybe I'm just not good, since 9 year old kids can do the hardest songs on Expert. To me it's still mind-boggling.
Old games were pretty hard as well, I don't even remember beating things like Judge Dredd on the Genesis, among others.
A few of the Hardest games to list,
Demon's Soul.. Beat the very first level (after the tutorial level) the first time, and can't do it for another few tries just for getting more souls but instead losing them all...
Rome total war, quite manageable in the early part but too hard to retain your own settlement without peasants revolting in the late game. So it's hard. Not to mention Medieval 2 total war which is a nightmare compared to Rome.

Same! I started campaign a few times all over again to get the same result... for me those egyptian chariots riding around my doorsteps~! Revolts double the trouble.
Rogue Squadron II on the Gamecube...I wonder if the guys at Factor 5 actually bothered to play it from start to finish, some levels
are impossible to beat without cheat codes and/or replaying the first easier levels ad nauseam just to get the maximum amount of lives...!
Quite few actually, mostly the ones from NES/SNES era, Contra 3: The Alien Wars for instance, which I never have really beaten. Few games on my GB gave me trouble, Spiderman and Gargoyle's Quest in particular, now when I think about it, older games in general were quite challenging.
Out of the newer ones though, I'll have to say Street Fighter IV, Seth's challenging alright, I'm sure there are more games but my memory doesn't serve me well.

Sagat was easy, actually. ;) I didn't have a hard time with him. Still don't to this day. :lol:
The hardest videogame I've ever played? The Ninja Gaiden games are right up there. Battletoads was also hard...
I can't recall a single game that was too hard for me. Difficulties weren't that punishing in this decade, but they were hard back then, as in the 8-bit and 16-bit days.
