Probably any one of the Final Fantasy's that use that FF music that has been used in several of the games (I love that music). I can't remember which ones the music is used for on the game over screen but I know there is at least one.
Everytime I hear that, I just want to burst out laughing :D my favorite as well. Metal gear solid's full of jokes that appear ever so often, that isn't one, but it just comes off funny.
Resident Evil's You Are Dead is a classic, also, in Viewtiful Joe, when you die, the screen darkens and you hear the director yell "Cut, cut, cut!" That's pretty cool. BTW, I saw that screen alot in Ultra V Rated.
I'd say the one from Donkey Kong Country, if only because of the music. One of the only game over screens that truly makes me feel depressed for dieing.
I like the one from Prince of Persia: Warrior Within, it's very to-the-point. :Prubber-chicken
It really rubs it in though when you get the "Game Over loop" glitch. When it asks you if you want to continue, you say yes, but then the Game Over screen comes back. You say yes again, it happens again. After about the fifth time, it let me continue.
I still have nightmares about the flashing lights and gannon's laugh. I hated dying even more knowing I was going to be subjected to that game over screen.
