An interview with the designer of the game revealed that the game bases it's probabilities on a random number generated at the start of a mission.
The chance to hit is determined by a number of factors - aim skill, weapon, range, cover etc. and shown to you as for instance 60% chance to hit. That is checked against the fixed random number and so either always hits or always misses.
OK so you have reloaded because your soldier missed a vital shot, and every time you take that same shot he always misses - what can you do?
If you can move and shoot in the same turn, move that soldier to another position and take another shot - the randomness is not guaranteed that that soldier will always miss this turn, only from his original exact position - 1 cell left or right is a different random calculation and one that might work out for you.

As you say, it seems that it is fixed...however, when shooting with a rocket launcher, it is not fixed. I had this happen when I saved right before entering a UFO that I had shot down. I saved before taking the shot on a group of aliens, and hit 2 of them. I used my other soldiers to try and capture one alien, but I failed in doing so, and the Alien turn killed off 2 soldiers.
You can imagine I wasn't happy with that, so I had to load my savefile. I made the same rocket launcher shot, and I missed???
So, my conclusion, those are the only ones you can actually do over until you are happy...regular shots I'm not too sure off, altho I think maybe every once in a while you can actually get a different outcome, perhaps by having an other soldier move first (get behind cover, use overwatch, whatever) and then try and take that same shot...haven't had to test that tho...
