I read about a mod that changes the way the game scales , the way loot is handled and also makes everything a ton harder. Someone compared it to dark souls on skyrim. Does anyone know the name of this mod?

I don't use just one mod for all of those effects.
I use :-
1. Deadly Combat (Improved combat)
2. Morrowloot (very little randomized loot and more unique loot)
3. Enhanced High-level Gameplay (mobs scale past lv 30)
4. ASIS (improved enemy AI)
I don't know of a mod that does all of those in one go, though.

High Level Enemies.
I use this, and have to reload several times. Best combined with Deadly Combat/Duel Realism/SkyRe Combat module, and ASIS.
It makes enemies level up according to player, and even higher. There are some optional files, like Hardcore, to make overall enemy's level higher.

For me, I use
Deadly Dragons. atm it wants Dawnguard, but I use an older version that doesn't need Dawnguard and it may be available. What this does is allow you to scale certain aspects of all Dragons, actors, and creatures.
Damage they deal
Health
Stamina
Magicka
This way you can even have difficulty on normal so that you deal good damage, yet have enemies that deal tough damage to you. Since I play a mage with magic resist I put dragon breath damage to 200. So they can kill me very quickly.

I'm having a blast playing with requiem + deadly dragon. It makes skyrim totally different experience, even harder than dark souls imo. I strongly suggest to play with mod's default difficulty without adding any tolerable options like vanilla stats at level up, requiem only let you have 5points. Also don't forget to add follower mod like UFO because you'll need more than 1 follower for requiem, at least for beginning of the game.
