You first need something to breathe with, a Gas Mask, Scuba Gear or Oxygen Tank will all work.
Then you need to pick up a rat and give it to the doctor.
Then you need to give the doctor something a doctor might use, a Health Potion, Syringe or Stethoscope will all work.
Then you need to walk to the girl, click on her and choose 'give'.
