Does an alert in Snake's Metal Gear Solid 1 flashback count?

It does not. You can kill, get alerts, and even die in the flashback, and it will not affect your rating in the slightest. (Not sure about time, though.)

Yes it won't affect it happened to me before (alerted) in the flash back but it didn't count as it happened to me before and it is explained as that you was alerted in the past not on your current game play -_-,Hope I helped.
