You can miss it if you don't periodically explore the Citadel after each mission.
Oh, and she's found in the Presidium Commons, really close to where Liara is seated although I believe you can only get the scene somtime during the first half of the game before it disappears for good.

It might be ME2 import dependent, but you can indeed meet Liara's father at the cafe in the Presidium Commons. She's the asari behind the bar. Voiced by Claudia Black, no less.

I don't believe it is, although I think you might've had to import a save from ME1 where you helped out the Asari bartender in Chora's Den on the Citadel.

Definitely NOT Lotsb dependent, I never got it either.
I am dating Liara, and talked to her father extensively on Illium in ME2, not sure if either of those matters.
