SPOILER Why couldn't i use paragon om TIM?

So i completed the game, with every sidequest done the paragon way, achieved a full paragon meter, but when i met the Illusive Man at the end of the game, i couldn't use paragon at the final dialogue. It is available the first 3 or 4 times, but the final one is still gray. Am i missing anything? I haven't played multiplayer, so i only have 50% galactic readiness, but i still have about 3800 EMS.*

Also, why didn't i unlock the final scene where Shepard breathes after the destroy ending?
