Go into the 4th quarter trailing by at least 7.
Score by throwing with your QB.

I believe you have to enter the 4th quarter trailing in score, then score to take the lead and win the game. Possibly, being ahead in the 4th quarter, but giving up a touchdown to the other team, then scoring yourself again to go ahead for good will also count, but I'm not sure.
