Id say get the prepaid card for LIVE, keeps your personal info off the net, and if u use a credit card it can renew itself automaticly (lucky i caught it and called MS) got my money back but its easier to control.
u don't have to go to a gamestop, best buy, etc. have them too, get urself a $50 gold subscription card or watever and u get a year, and u don't have to buy another card after ur year is up if u don't want to, its just up to u

Yes - you pay to play online. System link is free, but of course requires you to have 2 consoles, TVs, copies of the game etc. Games that support splitscreen are free to play with several people as long as you have enough controllers for everyone.
