Kingdom Hearts, from my own experience is known to have the hardest optional bosses. KH3D's Julius, I have yet to beat. And the Mysterious Figure on Birth by Sleep I never beat as well.- Please wait. Quick reply will be available shortly.

Demifiend (SMT: Nocturne protagonist) in Digital Devil Saga. I never beat him because it's almost impossible unless you're incredibly lucky and have all the highest gear.
The one that I actually beat is probably Yiazmat in Final Fantasy XII.
