Well, to be fair, nopony by the name of Ditzy Doo (officially) was ever shown on screen. I think it was really that people likened her derp eyes with the name, and also her referred clumsiness in winter wrap up, so everone just assumed and decided that that was her name.

The name Ditzy Doo was mentioned in Winter Wrap Up, when a pegasus was lost trying to help the birds back home to Ponyville.

Derpy was nicknamed as such because of a wall-eyed moment during the party Pinkie holds for Twilight in the very first episode, at around 15:08 into the episode.

Lauren Faust has stated:

That episode was made after Lauren Faust quit the show, and after fans discovered her.
