There was one puzzle in Penumbra: Black Plaugue involving computer servers that was excessively complex
Played a myst-like game called Lighthouse around 1997 (or so) that had this steam pipe puzzle at the end. It took me a very long time (like 3 days of several hour or two sessions) trying to figure it out. I vowed I wouldn't use FAQs and finally finished it. The ending of the game wasn't that good :/
Trying to enter the keypad to get into the docks on Jurassic Park: Trespasser, due to the arm not going where I wanted it to :P especially trying to manouver it when I have a T. Rex and Triceratops battling their way towards me down a hill, it was pretty cool though.

At least that one was logical and fun to solve - basically a do-it-yourself Da Vinci Code, before The Da Vinci Code was even written. The cat-hair-and-maple-syrup moustache puzzle, however... :(
Anyway, the prison gate puzzle in Riven was probably the most complex I've ever seen (while still remaining logical and believable in the game's setting). To say anything about it would be giving it away, but it involved gathering a number of visual and aural clues which were cleverly and quite naturally located all over the game's various islands, with reference to a fictional number system which you had to teach yourself.
It was all rather excessive, really.

Thats a tough one! I'd like to say I solved it but I never did fix those periodical BSOD's. You are a better intellectual than I, sir.
I usually dislike puzzles, but sometimes they're fun. The hardest one was probably one of the time-manipulation puzzles in Braid. That game was tricky.
In KoTOR I remember having a hard time on my first playthrough solving the puzzle in the Rakatan basement and also the puzzle to open that freakin' door in the Tomb of Naga Sadow(?).
I'm not sure about the hardest puzzle, but Machinarium was a damn hard puzzle game :x

Made me Lol NEw Vegas actually not Blue Screen crashing my Comp would be a great achievement for me......Lol speak of the Devil!
For me i remember Silent Hill 2 or maybe 1 throwing a few Curve balls when i was young remembering the specifics aint going to happen tho. Baldurs Gate 2 had some good Riddles which i enjoy to be fair. Penumbra had a few in that were quite hard. Ohhh and AC2 revealing the hidden video one of the Puzzles i just could not do at all had a number wheel in it etc!
Braid. entire world 6 where you use slow motion bubbbles or something. i've spent hours on that world because it took me a long time to figure it out. i think my mind is weak. lol :P

It wasn't the hardest puzzle, but solving the morse code puzzle in Penumbra without any walkthrough made me very happy.
