PC Game with good Story and unlocks/Collectables??

I am looking for a good PC game , RPG if its out there that has: Good Story, Lots of Unlockables, Lots of hidden things to find, Lots of side quests to complete. Maybe even something where you get graded on the side quests so there is a good reason to replay them. Like Just Cause 2 and Saints Row games, where there is a lot of things to collect, Unlocks and Exploration. Any ideas? Doesn't need to be a RPG.

Planescape: Torment. Lots of side quests of incredibly good quality. Lots of hidden stuff to find. Every single piece of dialogue is excellent, and it not only has a good story, but it also has some of the best characters i have ever seen in a video game.

The User Interface is clunky, and it plays like an adventure game - Explore, talk to people, solve puzzles, etc., but it's worth playing because there is no modern equivalent that comes close to its quality. The graphics are beautiful in HD resolution aswell.

Not too many great 'story focused' (writing/characters/dialogs/setting..) RPGs. But yea, Planescape is the obvious choice.

You also have other good ones like...
