Joaquin Phoenix's Joker. Don't get me wrong Heath Ledger was good, but he was more of an anarchist than straight-up insane. More of a mad genius than someone with mental health problems. Phoenix's Joker was actually mentally ill and it was done well.

Split/Glass. Say what you want about M Night Shamalamdingdong but James McAvoy played a really interesting and scary crazy dude.

Henry. Played by Michael Rooker (brilliantly!), this is a really good movie about serial killers.

Silence of the Lambs. I don't really need to explain this one.

Antonio Salieri from Amadeus... not sure he's psychotic except thinking Mozart is God talking to him, and plans to enact revenge on God for not blessing him with ability to celebrate Him through music, so plots to kill Mozart as means of revenge against God, I mean, seems some kinda crazy.
