It's a nice little puzzle game.

Dr. Mario Online RX is Mario's first Wii Ware offering, coming in his doctorate puzzle form. The gameplay is exactly like that of previous games: line up pills to destroy viruses, matching four of the same colors in a row horizontally or vertically. The speed at which the pills fall comes in low, medium, and high rates. There is a virus buster mode which takes advantage of the Wii Remote to drag the pills into place. It's also the first in the series to go online. The setup is very simple, though. You start with 5000 points and your points increase or decrease depending on how well you play. It's like Mario Kart's online, only a little watered down. You can also go against the CPU in regular and flash modes (where you try to destroy only the flashing viruses). It costs 1000 Wii Points, so if you have $10 to spare, then this is a good way to spend some of that loose change.
