    At one point in the game, Calista will be sitting in a booth in the Hounds Pit. If you talk to her, she asks you to find Emily. One time Emily was hiding beside the tower that is permanently locked up. Once I found her, she gave me an item that she says she found in the mud near the edge of the water. I wonder why the Heart didn't locate it?

    They're ignoring game design so you can actually play a little hide and seek.

    Or, having a rune in your pocket makes it undetectable, as opposed to in a safe or underwater or in a chest...idk, nanomachines and Mass Effect.

    Quest rewards/NPC items don't appear to the heart. One of the earliest examples is the pair of Runes you get from Granny Rags in the first assassination mission. You can find the one in her shrine, but have to earn the other two from her.
