What counts as detection?

Detection is when you see the lightning bolts around NPCs' heads go red, even one of them. Shadow Kill only works on enemies you haven't alerted, so yeah if the body didn't turn to ash you were spotted. The trophies are really finicky, so it's better to be safe than sorry. Save often and when in doubt reload and try again.
