Nope you cannot kill a single soul..also look out for packs of rats they will eat sleeping soldiers and that disables the achievement also. Take all the nonlethal objectives every time during missions also.

Rats don't eat sleeping soldiers....even if they do, if you're playing a Clean Hands playthrough, you shouldn't get rats anyway.
To answer your question, to get clean hands, you must kill NO ONE. No wall of sparks, no unintentional suicides. Nothing.
