Where can I find all of the heads?

There are a whole slew of heads out there, and we all want to know how to get them all.

How to unlock:

- Defeat any Badass enemy
- Complete mission "Symbiosis"
- Complete mission "In Memoriam"
- Complete mission "Statuesque"
- Complete Challenge "... To Pay the Bills"
- Defeat the hidden enemy in Caustic Caverns
- Defeat the boss of mission "Best Mother's Day Ever"
- Defeat the boss of mission "Where Angels Fear to Tread"
- Defeat the first target of "The Talon of God"
- Defeat the second target of "The Talon of God"
- Defeat the target of "You. Will. Die. (Seriously)."

The main ways of getting heads are:

1. Complete certain quests (usually side quests)
2. Random mob drops or from smashable items (the latter much more rare)
3. Certain bosses and creatures have a high drop rate, but usually only for a specific type of mask... read below for a spoiler if you wish that gives an example...

When you kill Handsome Jack he drops a head cosmetic that is a mask of himself.
